FAQs
Do you accept any insurances?
1
I am in-network with the following plans: Cigna, Premera, Regence, Lifewise
What if you are not in-network with my insurance plan?
2
I do not accept insurance plans for which I am not in network. However, I can provide a superbill/detailed receipt that some plans are willing to accept and reimburse at least part of the cost of the self pay rates. I would recommend contacting your insurance plan to determine if they do.
How can I get scheduled for an evaluation?
3
Check out my “Contact” page. All communications go through my assistant. Their contact information can be found here.

What can I expect at my evaluation?

What should I wear to PT appointments?
Expect to discuss your issue, pertinent history, and goals.
Expect manual and movement testing of tissues, joints, and body systems.
Expect to move! We’ll look at how you move with a variety of functional movement screens.
The ideal would be shorts and a sleeveless top. I can assess most effectively when I can fully see the movement patterns of the joints and muscles.
That being said: wear only what you are comfortable with in regards to level of visual exposure. The only firm requisite is that clothes be comfortable and free flowing enough for full, unrestricted movement.
